Got too much neck relief.
I have been trying higher and higher gauges over the years. I am now at 45-109, which is perfect for me. But with the action set my desired height at 8/64-6/64 and some concaveness of neck, I am getting flat notes between 5th and 12th frets.

I took off the neck, gently tried to turn the nut to no avail. It will easily turn the opposite direction, though. My auth. serv. guy is on the mend and can't help me right now

If the nut won't turn and you have tightened it before to the point where it is at and it isn't flat--- and if lowering the saddles doesn't help---- I would take it to someone(repair guy) and have it checked out before I tried anything else. But that's just me.

If the rod will not tighten any more and the neck is still concave, you can install a rod shim, which is actually just a washer or two under the truss rod nut. This repair involves taking the rod nut off and placing the washer underneath it. This will give the rod more adjustment capability and your neck should straighten if there are no other problems with the bass.

Good Luck.

B.C. is giving a solid solution to this. Any good luthier can do that. Personally, I might want more than 2 washers though to give you more adjustment should you need it in the future.

If adding spacer washers does not fix it then you might need the neck heat pressed. That is a difficult service to find and you often have to send the neck off for that. Of course one thing you do not want to do is strip the threads on either the nut or rod, so if it won't turn easily never ever force it. Good luck.
