I bought a brand new USA jazz bass two years ago and had to add washers to the truss rod to get enough adjustment, it was maxed out before the desired neck relief was attained. Can anyone give a reason why a bass straight from the factory would be like this?

I suspect poor quality control on their part. I had a serious issue with the neck on a Squier Bass VI, which I had ordered and waited back ordered for, when those first came out, and the neck was completely screwed up to the point of being unusable, and I had to send it back. It actually came that way straight from Fender, brand new in it's box, so whoever put it in there from the factory was the last person to handle it before me. You'd think USA Fender would be a little better about that, but perhaps not.

I don't think the true cause can be known without pulling the truss rod from the neck and measuring everything. There are several possibilities, but they're nothing more than that, and so their usefulness is roughly zero. If what you're doing today to make the bass serviceable is working, stick with it.

I have a situation with an American Standard P where I need to run the G and E bridge saddles almost all the way down to the deck for the action to be decent (not super low, just decent) -- and yes, the neck is straight. But the bass plays well, it feels perfect in my hands, it's stable, and it's my main player, so it's all good.
