If you pull the jack, you can probably identify it visually on a parts website.
Try mojotone, ampwares, or Antique electronics.com

Had the same exact issue, found an open solder joint at input plug, trace was cracked so ran jumper to pin 1

power switch in my amp fell apart a few weeks ago
