What speaker cabinet is it hooked up to? What's the cabinet's impedance rating? Is it stereo? Is it Mono? Have you checked the input voltage?

A lot of things need to be checked first.

If the jack plate is metal, take one of the jacks off of the plate and hook the inputs back up and give it a try...
I did this and all was fine, so I made a jack plate from plastic and it is working great..

I have a 2 x 12 and hooked the speakers each to a single jack that were both mounted on a metal jack plate. The amp made a hella sound when I turned it on. So I took off one of the jacks from the metal plate and then it worked ok. It must be grounding out with both jacks mounted on the same metal jack plate.

I did not test it with another amp, but it did make my Mustang V work.. BTW, The first Mustang V, I had died in a couple of hours and I was just using the headphone out. The one I got as a replacement has been jamming well..

Most guitar amp heads are mono and only require one speaker lead to make the cab work. The Mustang V is and always will be a stereo amp, requiring TWO separate speaker leads to a single cabinet (or two separate cabinets) to work properly. BOTH of the speaker outputs from the M5 are 8-ohms, so each side of your cab needs to be 8-ohms a-piece (8-ohms for the left channel, and another 8-ohms for the right channel). If they're not, that could be your problem, especially because you say it happens when you crank it up (increase the signal going to the speakers, which directly affects impedance). You should probably check that.

My JVM has multiple outs, but I use the 1960A cab at 16-ohms. If I were to hook it up to the V412 (Mustang V cab), I'd use 2 x 8-ohm jacks for a 4-ohm total. See how these numbers can get confusing if you aren't paying attention?

I had the same problem when i bought my mustang V head back in march 2011,i had it hooked up to my line 6 spider Mono/stereo 4x12 cab,the line 6 cab would be 4 ohms mono with one 1/4" speaker cable or 8 ohms stereo with two 1/4" speaker cables hooked up but it did not work with the stereo connection from the mustang V stereo outputs.

This was kind of strange because the line 6 4x12 is a stereo cab but must be wired different then the mustang V 4X12 that is also 8 ohms stereo?

I think the line 6 4x12 shares the same ground for the left and right channels and the mustang has a separate ground for each channel,in my opinion this is what is causing the problem.

FFXIhealer is right, the connections must be wired separate for each left and right channel, that is what i had to do with my line 6 4x12 cab,i removed the small circuit board with the two 1/4" input jacks and put in two new 1/4" mono 2 conductor jacks and wired each right and left side of the internal speakers + - wires separate to each 1/4" input jack.

Note: the jack plate on the rear of my line 6 cab is plastic and i did not have to insulate the 1/4" jacks,if you have a metal jack plate you will have to use non metallic type washers or a piece of cardboard and or plastic to insulate the 1/4" jacks.
