Imput jack is very simple to wire. Bridge ground ,an Hot wire should be the center post on the volume knob.

Look up how to solder electronic equipment on the web.
You'll need the following stuff:
Work place and a towel to lay the guitar on while performing "surgery"
Safety Glasses to protect your eyes from poping solder (it's rare but it does happen).
A # 1 and #2 phillips screwdrivers.
1 Output jack assembly; this includes the jack ferrule, mounting screws, jack, hex nut and flat washer.
1 set of strap buttons or strap locks of some kind.
1 soldering iron; available at wal-mart like stores, hardware or electronic supply houses.
1 Small pack of rosin core solder. The rosin cleans the wiring joint as you go; otherwise the solder will not flow into the joint; you'll see what I'm talking about when you start soldering.
Knife or wire strippers.
1 towel to put under your guitar to priotect it.Wash cloth or rag for use as below.
All the joints you'll be working on only need about 3/8" or so of the wire's insulation removed so you can join the connections. Use your knife or stripper for this.
A wiring diagram of your guitar or one like it. Available on this site and numerous others.\
Some sort of tool rest for your hot soldering iron to keep it from burning your furniture, guitar, you, or other things nearby.
Get some wire about the size of school pencil lead; # 22 or 24 maybe (not sure on thses sizes; but you might find the size stated on this site or others in the wire diagrams or parts lists.
Reccomend you practice soldering/stripping and un-soldering (just heat the joint up and gently pull the wires apart) with this wire before you start your project so you'll know what to expect. Some folks twist the ends of wire before soldering; that can make it hard for the solder to flow into the joint if it's fine wire and this wire is fine.... so just make sure the wire is as it appears when you strip it with no strands outside the joint. You may have to twist wire to wire joints together prior to soldering just not REAL tight; just enough to hold them together.
Here we go:
Plug your solding iron in; it'll get hot so take nesessary precautions included in the paperwork that came with your soldering iron.
Assemble the output jack and the ferrule.
Place your towel under your axe and your rag over the guitar in such a way that it'll protect the finish from accidents like dropping the hot soldering iron; dripping solder.
In the hole for the jack you should see 2 wires; white and black I belive.
The black one is ground and the other (regardless of color) is the "hot" or signal wire.
The hot wire gets soldered to the lug that is connected to the long prong on your output jack. Just look at the jack and you'll see what I' mean.
The ground is connected to the the remaning lug.
Form a "J" shape with the wire ends you're going to solder to the jack "tin" these ends; that is to say to apply solder to the ends creating a solid terminus; this makes it easier to complete the job. Loop them through the hole(s) in the lugs is such a way as they won't fall out before you get the solder and iron to them. not tight but loosely
Now heat the LUG and wire simultaineously; having your solder at the ready; heating should take less than 1 minute; then stick the end of the solder to the lug/wire junction and it should flow over the lug and wire creating a shiny appearance; when all is nice and shiny remove heat and your solder. There you have soldered a joint! Do it all again on the remaining lug. Usually you'll find that the hole in the lug is closed with solder; that's PERFECT!
Plug in your guitar and see if you get sound from your amp; if so install the ferrule.
Screw in your strap button(s) /locks
You're done.

You can find all kinds of parts and wiring digrams on the internet. As long as the neck isn't warped or anything, it shouldn't take too much to bring this guitar back to life.
