Keep in mind, that's mainly how it is here in Vegas. I can't speak for other parts of the Country.

It's the same production standard here in southern Arizona. In the home I bought back in '87 there isn't a room, wall, doorway, or window casing that's square, level, or plumb. I've spent the last 27 years fixing what I could and learning to live with what I couldn't. When we had a 360 square-foot addition built three years ago (to finally give myself a proper music room) I drew up my own specs, watched the contractor and his crew like a hawk, and documented the entire process via photographs. My diligence resulted in an addition that's built three or four times better than the original structure it's attached to. And in another year or so my backyard lawn will have finally recovered from the trauma.

That doesn't surprise me really. Although I'm mainly doing commercial work right now, I did residential for many years. Before the economy crash, I was doing stair railings.

This was a fun one. The rail does a 45-degree turn at the bottom. Since there's no real way to do that by just cutting the rails, I had to bolt 3-rectangular blocks together in the air and carve the rail by hand using grinders and sanders. The end result is pretty crazy. It all has to stay in code in every spot.
