Common mistake #1:
The Reverb is obvious

My opinion:
If your non-playing spouse can tell the reverb is on, its too strong; set it on 1 or 2--that's enough. Exceptions: when Reverb is "the effect": surf music, ambient/new age,...

Any other tone mistakes you've seen "in action?"

:? all am matter of opinion, it all depends on if you are going for that effect. like for a nice slow soft intro or long tail off. there is a Kings of Leon song " cold Desert" that is quite nice with tones of guitar re-verb.
to me the biggest tone mistake made today is to much Excessive squealing gain. no tone there just buzzz chug chug buzz :lol: but if that's the sound you want than its useful.

In my opinion, the more reverb the better. I also rarely play without delay activated.

Cutting the mids way back on you amp. You'll never cut a mix like that. Metal guys are the biggest offenders.

Using way too many effects.

Using way too many effects together all the time.

Not utilizing the tone and volume controls on your guitar.

Thinking a Wah pedal sounds great in every song.

Don't mean a thang if it ain't got that twang! You know, when and were appropriate. Which is "almost always" for some of us. I'd say having a piercing treble is the worst tone mistake, the difference between being heard and being hurtful.
