Mine was the time that I forgot to put the clamp on my bass drum and the skin went flying and hit my bassist in the face!

I'm not a drummer. I was just messing with my drums during practice. I've been playing guitar for 28 years. Drums? About 2 years. :P

By the way, it's not Eddie Van Halen, it's EddieVanHelen, my wife's name.

Didn't check batteries in distortion pedal and played live in front of 50 + people, and being extremely inexperienced at the time, had no sound coming from my amp halfway through the set because battery died. I had no idea what went wrong. I looked like a goofball in front of a lot of folks.

One time I was playing at a large club and I tripped on my cable and my amp fell over.

Ah yes ! I've remembered one. I once played "Shotgun Wedding" without thinking about anything at a gig, [ Well it was on my list ]
Then suddenly realised it was a wedding reception.
Got bounced out of the place.
Ah well.

Only for a short while, I mean they could have had a sense of humour about it but, well no. Never mind life goes on. In my time I've done lots of things. Eh! so what.

No Eddie, When I started out, right back in the first gigs I did, I was embarrased just walking on to a stage. But as the years pass, all that kind of thing falls away. Have you been playing long Eddie?
