I've searched this before and found the information available is sketchy at best.
Here are a few bits of info that I'm pretty sure are correct or at least close to it.

Original "Wide Range" humbucking pickups, designed for Fender by Seth Lover, were available as early as 1971 I believe (don't hold me to that) but they were usually placed in the neck position of some Strats but mostly Teles. A number of artists modified their Strats with PAF buckers in the late 60s and early 70s, including Joe Walsh, Alan Holdsworth, Micheal Hampton, Dave Murray, Vince Martell, Bob Rasmussen and others.

As far as the first production Strat to be HSS, I think the first one I know of personally was made in 1986, called the HM Strat. It was generally considered to be Fender's response to the Japanese companies that were selling "SuperStrats", which were based on Strat designs but with hot-rodded components and are still available today and widely used by shred style players.

Hopefully someone might chime in with better info as I've long been curious myself.

Just think of how awesome a guitar player you could have been by now if you had only spent the last 10 years practicing instead of obsessing over pickups and roasted maple necks.

The following year, Ibanez introduced the JEM.

Since the HM Strat was HSS, I think the JEM was probably the very first production guitar with the single coil in the middle of 2 humbuckers (HSH). I am not aware of an earlier production model with that specific configuration.

That does indeed seem to be the first of what I could find too so far.
Never seen it.

There was a topic on HSS Strats a while ago: viewtopic.php?f=6&t=101748
And (entering acronyms hell...) BTW to BMW-KTM; the HM Strats followed the earlier Japanese Contemporary Strats, which had HSS models.

Edit: I'm guessing the first Fender HSH would be Japanese, too. I'll check & get back on this.
Edit2: The first Fender HSH I found is the STR-650 MIJ model from as late as 1990.
As a fun fact side remark, I also found a Squier HSS MIJ from 1983; the SST314H - the curious part of it is, it's a short scale in Gibson style (314mm x 2 = 628mm = 24,72" scale = ~24.75"...).
