Most importantly, he was using a volume pedal.

Phil Keaggy of Glass Harp pioneered that sound and that technique in 1967, the year before he joined the Harp.
He taught himself to hook his baby finger around the volume control of an an electric guitar while he plucked strings much like some guys hook onto a trem arm while plucking.
He would pluck with the volume rolled off and roll it up immediately after plucking.
Exactly what Vai is doing with a volume pedal but Keaggy did it with his hands.

Can't get that in an amplifier, though.
At least nothing that uses algorithms to do it automatically.

Phil continues to employ this technique as a solo artist. He's one of the most under-rated guitarists in the business.

I knew that was coming. :roll:

A Fernandes Sustainer pickup basically acts like a built-in E-bow. Effects and distortion help, but the pickup also functions without them. A volume pedal (or knob) will never increase sustain, although it might sound that was to an untrained ear.

The OP's question was about more than just sustain.
He wants the ability to sustain, yes but also to increase or decrease volume, mid-note, as a form of expression.
Like his horn player.
