I have owned this pedal for years, and have never seen another one like it. Has anyone ever seen one? I'd like to know what year its from, if I can get parts for it? Everything functions on it, but the high end of the wah crackles a bit. any info would be greatly appreciated! It looks really cool, and everyone who sees it says it must have come from the gas pedal on a 59 Blue Bird school bus.

It shouldn't be too hard to replace the pot in your pedal which is probably the cause of the crackling you're hearing. If you're not comfortable opening it up yourself, take it to your local amp repair guy (if you don't know who that is, take it to your local guitar shop) and let them deal with it. I wouldn't think it would cost a whole lot to replace.
