New to me. Never saw one before. I'm guessing custom made. The headstock decal is suspect as it just doesn't look right.

B.C. Bassman might know something.

Like brotherdave said, the headstock logo is not the correct logo. Tuners are odd too. Even the headstock lower hump looks too big. It looks like some crazy mutant telecaster guitar body, with a bass neck. I don't know.

Still trying to figure out the bridge and tuners came from though.

I think its a home made project bass that someone put together in their basement.
It is definitely old and it could be another Phillipine back alley factory Fender Fake but they usually tried to copy real Fenders, this is more of a fanatasy Tele bass in a short scale. Possible that it was made to order this way or sombody converted a Castilla Tele bass with a real Tele pickguard.

Those tuners look like early asian tuners that were on a variety of basses Maybe they are Hagstrom tuners, I don't know but they were available back then.

The bridge and bridge pickup are crudely made and not Fender but very likely of Phillipine origin. Back inthe 60's and Seventies , many fake Fenders were sold to servicemen who were stationed at the Subic Bay base in the Phillipines.

Back in the 60's- Fender would sell you decal if something happened to your guitar. Remember back then the decals sat on top of the neck finish. It was very easy to soften them and put them on another guitar. Easier still if Fender gave you a new one.

Fender didn't wise up to this trick until the early 70's.

I have taken apart perfectly copied Fender Jaguars with hand painted decals that had their entire wiring harness in one color of wire! RED!

I have seen Jazz basses with one pickup and two piece bridges hidden under fake chrome covers!

Most have opaque finishes -many sunbursts with solid yellow centers to hide the fact that the bodies are made of mahogany.

Most buyers of these guitars were fooled when they purchased them and were ripped off for serious money.
Fenders have been copied for years. Beware when buying used!
